Home Healthcare

Home healthcare is provided by the client’s primary care physician or a hospital doctor upon discharge for up to 14 days, after which care continues through the primary care physician. Home healthcare is fully covered by public health insurance and is intended for clients with health problems that can be addressed through visits from a home healthcare nurse.

Caregiving Services

Caregiving services are social services provided to clients with reduced independence due to age, chronic illness, or disability. The goal is to improve the client’s quality of life and, where possible, integrate them into society.

Ambulance transport for scheduled check-ups or treatment

 

The healthcare transport order for transport to a contracted healthcare facility, where the patient is to receive medical care, must always be filled out by the referring healthcare provider who requests the care. This order (form VZP-34) must be correctly completed in order for the transport to be billed to the insurance company and reimbursed. The transport must be indicated strictly for medical reasons, not social reasons, such as poor transport connections.

 

If referred by a general practitioner, only the nearest healthcare facility may be selected. If the patient wishes to be examined at a more distant facility, they must cover the difference in costs.

 

For scheduled follow-up examinations, the referring doctor at the healthcare facility recommending the follow-up will issue the transport order. However, the doctor may not issue the order for objective reasons, such as when the follow-up is planned for a longer period (6 months or more) and there is a possibility of changes in the patient’s health. In this case, the patient can turn to their general practitioner, who will assess their current health condition and, if necessary, issue the transport order.

 

The transport order for the return journey is issued by the doctor who provided the healthcare. They must assess whether the return transport to the patient’s place of residence is medically necessary.

 

If the patient prefers to use a private vehicle instead of an ambulance, and the doctor approves this option (by issuing form VZP-39), the patient is entitled to reimbursement for travel expenses. The condition is that the vehicle is driven by someone other than the patient, which corresponds to the patient’s health status that prevents normal transportation or driving.

 

Accompanying person

The presence of an accompanying person is indicated by the doctor if necessary, for example, due to patient disorientation or limitations in their ability to communicate.

Home hospice care is a health and social service intended for patients with incurable illnesses, where curative treatment has been discontinued, and palliative care has been recommended. The care is provided in the patient’s home, in collaboration with their loved ones, who are willing and able to provide round-the-clock care.
